Non-Binding Hospital Partnership / Referral Agreement
1. Purpose
This Agreement establishes collaboration between ReAssure Med and the participating hospital or treatment center.
2. Role of ReAssure Med
ReAssure Med provides second‑opinion coordination, care navigation, and patient introductions.
3. Role of Hospital
The hospital agrees to provide consultation, diagnostics, and medical treatment where appropriate.
4. Independent Treatment Relationship
Any treatment relationship shall exist solely between the patient and the hospital.
5. Patient Consent
Hospitals may receive patient information only after receiving proper authorization from the patient.
6. Compliance with Healthcare Regulations
Hospitals agree to comply with all applicable healthcare laws, licensing requirements, and patient privacy regulations.
7. Liability Allocation
Each party is responsible for its own services and liabilities.
8. Marketing and Transparency
Any patient referrals must comply with applicable healthcare regulations and ethical standards.
